The CDI self-assessment

One sheet. Ten minutes. One honest read on where the team actually stands.

This is the Cognitive Delivery Index workshop canvas, made walkable. Six dimensions, each scored on a five-level ladder from Absent to Substantive. Score the team as it is, not as it should be.

This is not a performance review and there is no pass mark. The goal is the same as the workshop's: an honest read, and one priority for the next ninety days.

The five levels
L1 AbsentNothing exists. The control is not present in any form. The team has not yet considered the question.
L2 InformalThe control exists by convention. It lives in conversations and assumptions.
L3 DocumentedThe control is written down. Coverage is partial and consistency uneven, but a document can be pointed at.
L4 CompleteFully documented and procedurally adopted. On paper, everything is in place. Auditors are satisfied.
L5 SubstantiveThe control produces observable behaviour. It is exercised, tested, and visibly shapes decisions. The control is real.

↑ The governance theatre gap sits between L4 and L5. Documented governance is not the same as governance that shapes decisions.

How to score honestly

Score the team as it is, not as it should be.

Fours get the facilitator's question: can you name a specific occasion in the last quarter where this control changed an outcome? No means three. Yes, with detail, may mean five.

If you are scoring as a team, score independently first. The disagreements are the data.
